For those clients that need ongoing, consistent support from PSS. In addition to the services described above, services may also include

• Assistance completing benefits re-certification
• Assistance completing housing re-certificationAdvocacy and assistance regarding medical needs and care
• Assistance establishing their Health Care Proxy, Power of Attorney and Will
• Other needs identified by clients and their social worker
